Thank you for your interest in being a foster parent with Quality Youth Services!

We recommend you have the following information available before you begin for you and any other adult with whom you are applying:
 

  • Current employment information

  • Estimated household income

  • If married or have any previous marriages, spouse name, and date of marriage

  • Information on any criminal offenses

  • Smoke detectors

  • Screenings for lead paint

  • Drinking water tests

  • Four personal references including name, address, phone, email, and relationship to you (not more than one relative)
